Payment Plan Terms and Conditions

• You agree to one time payment.
• Payment is required on the scheduled due date.
• Payments will be debited from the credit card or ACH/eCheck information provided in this agreement.
• Services may be suspended or discontinued if payment has not been received within 15 days of due date.
Payment schedule:
• A one time payment of: $14800.00
• Lump sum payment will be discounted at 7.5%
Services:
• Group Home Biz agrees to provide “Turn Key” services as described in the SOW or original agreement.
• Customer may cancel services by giving 30 days’ notice in writing.
Invoices:
• All invoices will appear in the billing portal.
• Payment history information can be accessed via the billing portal.
Assumptions:
• You have already paid the first installment for “Turn Key” implementation services.
• This option is made available as a payment plan for the second draw only.
